Click HERE to contact us via email, or call 541-981-2620 and ask for an appointment regarding the mens study.  If you are not yet a cardholder but are interested, we may be able to “save a place” for you.  We will have doctors performing consults on October 30th and November 12th so you could get the paperwork for your card signed at that time if you are unable to get your own MD or DO to sign.  You need to be pre-qualified to be scheduled for a consultation so, if you don’t have copies of your medical records, you can download our Release of Information form HERE, sign it and bring, email or fax it back to us or your doctor and get the ball rolling.  

Do you need the paperwork for your doctor to sign?  You can get it from us at Albany’s Canna Kitchen or you can download it at the OMMP website HERE.  All of the forms are available there, but you first need the Attending Physician’s Statement if you’re going to an appointment with your own doctor and asking them to sign.  The biggie there is that they must be a MD or DO.  If they won’t or aren’t qualified to sign, then you need to come see us.
