So you’d like to get your Attending Provider’s Statement signed…

All OMMP Doctor appointments are scheduled with a $50 deposit. Deposits are required to be paid within 72 hours of scheduling or the appointment will be cancelled. Your deposit will go towards the final balance of your appointment. Patients who make other arrangements for paying the deposit and end up not showing for their scheduled appointment will receive a bill for $50.

We offer two main types of OMMP Doctor visits:

A Standard OMMP APS Signing with Medical Documentation

($160 total) ($150 for Veterans)

  1. Medical records may be obtained by filling out and returning to us our ‘Release of Information’ form, or by submitting current medical records that you have already obtained directly to us. Appointments cannot proceed until this is completed.

All Minors 17 and under MUST submit medical documentation for review.

 Extended Evaluation APS Signing without Medical Documentation

2. If you do not have current or up to date medical records, you will need to proceed with an Extended Evaluation.
($265 total cost) ($255 for Veterans)

Any remaining ACKR Clinic balances are due on the day of your appointment.

​The state of Oregon has application fees as well. You can find out about them HERE. Application fees are SEPARATE from clinic fees.

*The doctor’s signature is valid for 90 days. Please be sure to apply within this 90 day period, as the signature will be expired after 90 days and no longer accepted by the OMMP. If you are outside the 90 day period, you will need to make another appointment with the doctor and pay the fee again.*

The OMMP (Oregon Medical Marijuana Program) requires you to have a “State issued ID”, and Patients must be a resident of Oregon. You will be required to prove your residency to the OMMP.

Cancellation Policy:

If for any reason you need to cancel or reschedule, please contact us as soon as possible. Deposits are non-refundable within 3 business days of appointment.​​​ If you have arranged to pay the day of your appointment and do not show up for your appointment, you will be billed the $50 non refundable deposit and then charged the balance in full as your deposit to secure a rescheduled appointment.

**Please note**
When coming to an appointment, it is best to avoid the train tracks at Pacific and Queen Ave SW. The later you are, the longer and slower the train. In fact, it often stops and sits. The best approaches to Ferry St SW are from Waverly to Queen Ave or 34th Ave SW to Ferry SW during appointment hours.
