We offer two main types of OMMP Doctor visits:
(1) Standard OMMP Attending Provider Signature with Medical Documentation
Standard Clinic Fee $180
If you are a Veteran, this fee is $170
if you receive SSI the fee is $120
*We must see Documentation before or at the day of clinic in order to receive any discounts*
Medical records may be obtained by filling out and returning to us our ‘Release of Information’ form, or by submitting current medical records that you have already obtained directly to us. Appointments cannot proceed until this is completed.
(2) Extended Evaluation APS Signing without Medical Documentation
If you do not have medical records within 2 years of your clinic appoint than you will need to proceed with an Extended Evaluation.
($265 total cost) ($255 for Veterans)
All ACKR Clinic balances are due on the day of your appointment. If you are mailing a check this needs to be accounted for as you cannot be seen without payment. The ACKR fees do not include the OR State fee in order for you to get your OMMP Card. You can find out about them HERE. State fees are SEPARATE from clinic fees.
*The doctor’s signature is valid for 90 days. Please be sure to apply within this 90 day period, as the signature will be expired after 90 days and no longer accepted by the OMMP. If you are outside the 90 day period, you will need to make another appointment with the doctor and pay the fee again.*
The OMMP (Oregon Medical Marijuana Program) requires you to have a “State issued ID”, and Patients must be a resident of Oregon. You will be required to prove your residency to the OMMP.

If for any reason you need to cancel or reschedule, please contact us as soon as possible. We also offer Telemedicine appointments for our clinic for those who would like to do their clinic visit outside of the office. Payment must be received beforehand.
When coming to an appointment, it is best to avoid the train tracks at Pacific and Queen Ave SW. The later you are, the longer and slower the train. In fact, it often stops and sits. The best approaches to Ferry St SW are from Waverly to Queen Ave or 34th Ave SW to Ferry SW during appointment hours.
